Choptank Community Health (CCHS) was incorporated in 1978, as a private, non-profit community health center located on the beautiful Eastern Shore of Maryland. In 1980, CCHS was awarded a Service Area Competition grant from HRSA, designating it as a Federally Qualified Health Center. Since that first health center was opened in 1978, CCHS has grown to include five family practice locations, a dedicated pediatric practice and four on-site dental practices, a robust school based health and dental program and a migrant program serving about 300 agricultural and seasonal workers on the Eastern Shore.

Our name comes from the Choptank River that runs through our service area. We are the largest primary care provider in our service area, serving 27,000 patients annually. CCHS is accredited by the Joint Commission and has received PCMH distinction.

Through our work with Studer Group, CHHS has made a commitment to expanding and re-defining our mission and vision to reflect our focus on excellence. In early 2016, the Board of Directors approved a new mission statement: "To provide access to affordable, exceptional care for all," as well as a new vision statement: "To improve the health and well-being of the communities we serve; to provide an outstanding care experience; and to be an exceptional place to work and make a difference."

Choptank Community Health has a strong focus on Population Health with Care Coordination and enabling services. Collaborative relationships with local acute care hospitals, managed care organizations and health departments have led to improved care pathways for our patients. To help us focus on access to care, we created a team to explore what access means to our patients, our community, our providers and our staff. The team's role is to analyze and improve work flow, develop specific communication strategies and determine patient needs around open access and expanded hours. The overall goal of the Access Team is to ensure superb access to care for all patients by providing them easy access to the offices for appointments at days and times convenient for them; to give patients the ability to send and receive communications via email, patient portal or other means; to improve office work flow to ensure CCHS has the capacity and efficiencies to provide quality care for all patients.

The team created monthly dashboards for each care delivery location. These dashboards include no-show rates, new patient visits, next available appointment, follow up appointments scheduled, number of visits, and unique patients. After analyzing the data, a deep dive was conducted into one of the health centers with results outside of the expected, and recommendations for improvement are underway.
