Citizens Memorial Hospital is a fully-integrated healthcare system devoted to exceptional patient care provided by leading physicians and a caring staff. Citizens Memorial Hospital is located in Bolivar, Mo., and serves eight counties. CMH offers extensive hospital services in a 76-bed facility and includes 36 primary care and specialty care physician clinics, CMH Cardiovascular Center, Carrie J. Babb Cancer Center at CMH, CMH Wound and Hyperbaric Center, Senior Health Center, ambulatory surgery center, home health, hospice, and rehabilitation services. Citizens Memorial Healthcare Foundation includes one assisted living facility, five independent living communities, one residential care facility, six long-term care skilled nursing facilities, eight home medical equipment locations, Health Transit and Homemaker Plus.

CMH has experienced phenomenal growth for an organization in a small community since its opening in 1982 and has expanded to provide healthcare services in surrounding counties. We have grown from an initial employment of about 79 full time equivalents (FTE) to 1,700 FTEs and from a first year budget of about $6 million to $330 million this fiscal year. A valued member of the CMH team, physicians are instrumental in providing exceptional care and helping to fulfill the mission and vision of the organization. CMH started with three full-time physicians and now have nearly 200 physicians and mid-level providers caring for our patients and residents.

Recognized for its innovative efforts associated with organizational growth and exceptional patient care, in 2010, CMH was awarded the Missouri Quality Award, which is modeled after the coveted Malcolm Baldrige National Quality Award -- the only healthcare facility in southwest Missouri to receive this celebrated award.

CMH was awarded the prestigious Davies Award of Excellence by the Health Information and Management Systems Society, and has been recognized for being industry leaders in association with its organizational patient electronic medical record (EMR) system. Further recognized for its exceptional achievement in the use of EMR, CMH was the first rural hospital in the United States to achieve a Stage 7 Award from HIMSS Analytics for a virtually paperless patient record environment and the most comprehensive use of electronic medical records. CMH was also named a Most Wired and Most Wireless Hospital, and was named one of the Top 10 Rural Hospitals in America. CMH is one of the 10 largest employers in southwest Missouri as recognized by Springfield Business Journal. CMH is a leader in southwest Missouri and was named four years consecutively as a Dynamic Dozen Award honoree and in 2011 was an Economic Impact Award winner by Springfield Business Journal.

In April 2010 CMH began a partnership with Studer Group in an effort to improve the culture to fulfill the mission and vision of the organization, utilizing techniques that were evidence based. CMH had implemented employee and patient rounding, but with the assistance of Studer Coaches, the tactics now have been enhanced to improve the benefit of rounding efforts -- not just from a quantity perspective, but also more importantly the quality aspect. Implementation of the Evidence Based Must Haves has proven to be beneficial to the CMH organization, leaders and staff as reflected in our improved scores.
