Community Health Care, Inc. (CHC) is a Federally Qualified Health Center serving over 37,000 patients in the Quad Cities of Iowa and Illinois. Our mission is to provide the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible. Although we are available to all patients seeking primary care, we maintain a focus on being a safety net for underserved individuals in our communities.
As the organization has grown, so have the services and access points. CHC currently operates in five communities with nine locations. The services include primary medical, dental and behavioral health, as well as OBGYN, pharmacy, lab, pain, chiropractic, WIC and other support services.
CHC prides itself on having a highly mission-driven and engaged staff. Most recently, Employee Engagement was measured to be in the 94th Percentile compared with all other healthcare organizations nationally. “The work we do every day and the barriers our patients encounter, requires us to have a compassionate staff equal to the challenge and motivated to break down those barriers to deliver on our mission,” says Tom Bowman, CEO.
CHC has also received numerous recognitions for the quality of care provided. We are a Joint Commission accredited ambulatory care center as well as an accredited Patient Centered Medical Home through the National Committee on Quality Assurance (NCQA). Most recently CHC was recognized by the Health Resources and Services Administration as a HRSA Health Center Quality Leader, where we were among the top 30% of all HRSA-supported health centers that achieved the best overall clinical outcomes, demonstrating high-quality across clinical operations.
